National 211 Epic Collaborative

Visionlink is pleased to support the National 211 Epic Collaborative with data exchange capacities funded by the United Way of Wisconsin.  These tools are now part of the core Visionlink platform available to all customers, and can support any 211 across the nation.

This SDoH solution on the Visionlink platform provides four key options, supporting different levels of integration:

1) Resource Directory Delivery. This provides a configured, formatted, and regularly scheduled feed of 211s' resource directory service and site information, allowing medical practitioners to access these valuable community services directly within their Epic healthcare software platform.

2) Referral Delivery. With this option, Visionlink activates the referral capabilities, allowing information about these patient-specific referrals to be sent to service providers for follow-up and coordinated assistance.

3) Closed Loop Referrals. Turn this on, and now information can flow back to the healthcare providers at the clinic or hospital with updates about status and follow-ups.

4) SDoH Utility: This option allows Visionlink to act as a utility, connecting HSIS-compliant resource directories managed on other software platforms with the Epic Healthcare software platform.  The point? You can connect with Epic regardless of the 211 software you use!
